Connections Across Care

The Resource & Referral Hub helps connect children and adults across developmental disabilities, neurodivergence, intellectual disabilities, special needs, behavioral health, mental health, aging, caregiver support, and related care needs.

Connections may include education and transition, employment, benefits, advocacy, community living, family and caregiver support, financial and future planning, recreation and social connection, professional services, and other community resources.

You Do Not Have to Figure It Out Alone.

      We help bring the need, available information, and possible pathways into view so the next step is clearer.

      When appropriate, we may follow up to learn whether the connection worked or whether another direction may be helpful.
