 Ways We May Help

Resource and Referral Connections Collapsed

We help identify community organizations, nonprofit programs, qualified professionals, advisers, and other resources related to the need. Connections may include home and environmental resources, education and diagnostic resources, family-support programs, essential-needs organizations, and other community services.

Navigation and Advocacy Support Collapsed

We help organize information, prepare questions, communicate concerns, understand possible processes, and connect with qualified assistance for systems such as DDD, ALTCS, Social Security, education, benefits, future planning, and related care systems.

The Foundation does not provide legal representation or make decisions for another agency.

Grants and Scholarships Collapsed

Individualized, application-based assistance may help address certain approved care-related gaps when Foundation funding and program capacity are available.

Assistance is reviewed according to the situation, applicable requirements, available resources, and Foundation approval. Funding is never automatic or guaranteed.

Lifeline Connections Collapsed

Lifeline provides resource and referral support for an immediate practical need affecting the continuity of everyday care. Its purpose is to help stabilize the situation long enough to identify responsible next steps.

Lifeline is not an emergency-response or crisis-intervention service.

 Navigation Begins With a Conversation.

 A Direct Care Kin Navigator™ listens to the caretaker’s situation, helps clarify and communicate the need, explains possible pathways, connects the caretaker with appropriate community resources or qualified professionals, and helps them understand the next steps.

Role and Availability Notice

The Foundation is a resource, referral, navigation, and gap-support organization. It does not replace government agencies, emergency services, licensed professionals, legal representation, medical providers, schools, or existing case-management responsibilities.

Every situation is reviewed individually. The Foundation does not guarantee services, funding, eligibility, referral acceptance, approval by another organization, or a specific outcome. Connections and approved assistance depend on the need, information provided, program criteria, community resource availability, available funding, and partner capacity.
